If you want to know how stuff like this happens ask who his Congressman is, then ask the Congressmans office for their phone logs. That's the riony of a Congressional investigation, they cause most of this sort of thing by leaning on the folks running the procurement. Look at the Current Tanker fiasco. Boeing lost on the merits, so now they are trying to win in Congress.

Accountability is clearly needed. Secretary Gates should step in and clean house immediately. Every individual who touched this contract, knew about this contract, and participated in the selection process should be fired for cause immediately. Furthermore they should be denied the ability to work for DoD in any capacity whatsoever.
This is gross negligence, incompetence, and a breach of fiduciary duty, public trust, and a gross failure in performance.
How could Secretary Gates or anyone in the chain of command stand for this kind of performance. It's no wonder we can't bring the Iraq and Afghanistan fiascos to closure.

Before I retired, every source selection I participated in had a PRAG (Performance Risk Assessment Group). When possible we would speak personally with the PCO and ACO on past contracts with offerors. After selection, but prior to contact award, an on-site preaward survey, in conjuntion with the local DCMA office, would be performed if our procuring activity did not have recent experience with the bidder.
Obviously there was gross incompetence or something else was going on. DCIS needs to look at it.

It is always nice to see Congress get wrapped around the details while ignoring the essence. Diveroli and AEY are bit players in a world of merchants of death, aka arms dealers. These people owe allegiance to no-one and will sell to all sides. A Congress that has imposed laws of ethics on US companies in their international trade should be asking a deeper question: why does the US Government work with these crooks at all ?

Why has no one questioned how a 22 year old could get an export license, a munitions dealer license or be vetted to operate in Iraq? Maybe the 22 year old has political ties to this administration and that is the "best value" used by the army to award this contract?
