The problem with these "revolutionary" methodologies is that they run smack up against the same iron walls: institutional stovepiping and individual wildcatting. Did TQM bring about agency culture-change? Excellence in Government? Knowledge Management? Not really. This is just another buzz word to provide trainers/consultants with jobs. After these events (and I have seen plenty of them) everyone goes back to their cubes and and realizes they are totally hamstrung by their ingrained agency culture that actually punishes "outside the box" thinking.

More consultants. More Power Point. More time wasted. More distraction.
Next year we will do it again.
No one could have predicted 9/11. Too many potential targets. A tiny, murderous gang of non-state actors is almost impossible study well enough to predict precise actions.
